Add items to collection-template pages that are exclusive to that page

Essentially a collection that has a product that people can opt into.

HOWEVER, this requires there to be an individual form on every page, because we need to integrate each pages form with a different MailChimp list, to automate sending out the guides to people.

How would we go about this?
